One Brow Is Higher Than the Other: What Causes One Side of Your Botox to Drop First
Unpacking muscle dominance, brow asymmetry, and how precise micro-adjustments restore perfect balance
Introduction
You’re sitting at your dressing table, applying your morning mascara, when you catch a glance in the mirror and freeze: “Hang on… why is my left eyebrow sitting slightly lower than my right?” Or maybe one brow is arching up like a surprised villain while the other is completely relaxed and flat. If you’ve ever had anti-wrinkle injections (botulinum toxin) and noticed your brows wearing two different expressions, don’t panic!
Brow asymmetry after treatment is one of the most common concerns brought to my Ongar clinic, especially during the first two to three weeks post-treatment. But while it can feel alarming when you first spot it, the science behind why it happens is really quite simple, and almost always very easy to fix.
Let’s pull back the curtain on lower-versus-upper forehead mechanics, explain why one side of your anti-wrinkle treatment might settle or kick in faster than the other, and show you why a small tweak at your two-week review is all it takes to get everything symmetrical again.
EXPLORE ANTI-WRINKLE TREATMENTS
1. The Forehead Tug-of-War: Elevator vs. Depressor Muscles
To understand why a brow drops or hitches upward, we first have to look at how your upper face moves. Your brow position is held in a constant, delicate balance by two opposing muscle groups pulling against each other 24/7:
- The Lifter (Frontalis Muscle): This is the broad muscle spanning across your forehead. Its primary job is to pull your eyebrows upward when you raise your brows in surprise. It is the only muscle in the upper face that lifts the brow.
- The Pullers-Down (Glabella & Orbicularis Oculi): These are the muscles between your brows (the “11 Lines” or Frown Muscles) and around the eyes (Crow’s Feet). Their job is to pull your eyebrows downward and inward when you squint or frown.
When we treat forehead lines with Botox Injections, we’re relaxing the lifting force of the Frontalis muscle. If one side of that muscle happens to be naturally stronger, or if the product settles slightly faster on one side, that tug-of-war balance temporarily shifts, causing one brow to sit a bit lower or higher than its partner. It’s as simple as that.

2. The 3 Main Reasons One Side Drops or Kicks Up First
If you’re noticing an uneven brow line post-treatment, it almost always boils down to one of these three physiological reasons:
| Symptom | Underlying Muscle Mechanism | How We Fix It at Review |
|---|---|---|
| Dominant Natural Asymmetry | One side of your face naturally lifts higher or has stronger baseline muscle tone. | Micro-dose adjustment placed precisely on the hyper-active side. |
| “Spock Brow” (One Arch High) | Outer tail of Frontalis was left active while the central forehead relaxed completely. | A tiny unit placed right above the arch relaxes the high tail back down. |
| Slight Brow Heaviness / Drop | Toxin was placed slightly too low or too heavy for that side’s natural bone structure. | Treating the opposing depressor muscle (Glabella) to allow the brow to lift back up. |
A) Natural Muscle Dominance: Nobody has a perfectly symmetrical face! Most of us chew on one side, sleep on one side, and express more animatedly with one brow. If your left brow muscle is naturally twice as strong as your right, placing the exact same dose on both sides might leave the stronger side lifting higher or relaxing at a different speed.
B) The “Spock” Effect: If the central part of your forehead relaxes completely but the outer edge (the tail) of the muscle remains active, that outer muscle works overtime. It pulls the tail of one brow up into a sharp, arched peak, resembling Star Trek’s Mr. Spock! The fix? A microscopic droplet placed right above that high arch relaxes it back down in seconds.
And.. Just in case you’re interested, you might want to check out my companion article: The Arching Panic: Why Your Outer Eyebrows Flicked Up Into a “Spock Brow”, where I cover this in a bit more detail. (It opens in a new tab so you won’t lose your place here).
C) Asymmetrical Settling Time: Botulinum toxin doesn’t bind to motor endplates at the exact same minute across both sides of your face. It takes anywhere from 7 to 14 days for the full treatment effect to lock in. One side often kicks in three days before the other, creating a temporary wobble that evens itself out by day 14.
3. Why the 2-Week Review Appointment Is Non-Negotiable
This is precisely why I insist on seeing my anti-wrinkle clients back in clinic at two weeks! Trying to judge or correct your treatment results at day 4 or 5 is a major mistake, because the toxin is still actively settling into the nerve receptors.
Once we hit day 14, the full result is visible. If one brow is sitting a millimetre higher or lower, we can make ultra-precise micro-adjustments:
- If a brow is arched too high: We place a tiny micro-dose directly into the overactive outer muscle fibers to bring it inline.
- If a brow feels slightly heavy: We can relax the downward-pulling muscles around the eye (the crow’s feet or glabella), freeing up the brow to float back upward naturally.
